Emergency Procedures

Each child is required to have all medical information and emergency release forms properly completed and on file.  Accident reports are recorded on our Accident Report Form and placed in your child's file, with a copy sent home to the patient.   When emergency care is warranted, we will:

1. Call 911 if necessary.

2. Contact parents/guardian

3. Attempt to contact persons listed on the emergency pick-up list.

4. Attempt to contact child's physician

Any expenses incurred under the above references will be the responsibility of the parents.
